Overview

REACH-HF (Rehabilitation EnAblement in CHronic Heart Failure) is a facilitated evidence-based cardiac rehabilitation and self-management programme for use at home. Jenny is primarily responsible for the administration of tasks to support the growth of the REACH-HF research programme and its implementation in the NHS and beyond.
